It is simple to become a RITC fighter. Get your meds done after Dec
1, 2009 and your AZ license will be good through Dec 31, 2010.
You will need:
Physical - download forms below
Opthalomogical Exam - download forms below
HIV - just need lab report ( no form )
Hep B & Hep C - just need lab report ( no form ).

will perform all three tests for $129 total and will provide them the results in 1-2 business days. The labs are open Monday – Friday but Saturday appointments can be made by calling them before 5pm on Friday. The fighters must mention that they were referred by “MCDPH” to get this price.
